How do I add Work History in a Caregiver profile?

The Work History tab stores information about a Caregiver's previous employment. Follow the steps below to add Work History to a Caregiver's profile.

1. Click Work History


2. Click New:

  • Eligible for Rehire: check if this Caregiver is eligible for rehire at this facility.
  • Facility Name: enter the name of the facility.
  • Address: enter the facility address.
  • City: enter the city where the facility is located.
  • State: select the state where the facility is located.
  • Country: if required, select the country where the facility is located.
  • Phone: enter the facility phone number. If the number is international, check the box next to International.
  • Fax: enter the facility fax number. If the number is international, check the box next to international.
  • Contact Name: enter the name of the contact.
  • Contact Phone: enter the contact's phone number. If the number is international, check the box next to international.
  • Contact Email: enter the contact's email address.
  • Can we contact this employer?: check if you can contact this employer. Use the Comment field to enter additional information if needed.
  • Position Held/Specialty: enter the position held at this facility or the specialty.
  • Present: select Yes if this position is where the caregiver currently works, or No if not.
  • Date Employed From: enter the date the caregiver began working at this facility.
  • Date Employed To: enter the date the caregiver stopped working at this facility. If yes is selected for Present, this field will be grayed out.
  • Shift: enter the shift the caregiver worked at this facility.
  • Teaching Facility?: select Yes or No if applicable.
  • Supervisor Name and Title: enter the work supervisor's name and title.
  • Was this a Travel Assignment?: select Yes or No if applicable.
  • If Yes, Specify Agency: enter the agency the caregiver previously worked for.
  • # of Beds in Facility/ in Unit: enter the number of beds in the facility or in the unit.
  • Average Patient Ratio: enter the average number of patients the caregiver worked with.
  • Charge/supervisory experience?: check Yes or No if applicable.
  • If Yes, How Often?: enter how often the caregiver worked as a charge or supervisory nurse.
  • Reason for leaving: enter the reason the caregiver stopped working at this facility.
  • Salary: enter the salary the caregiver earned while working at this facility.
  • Work History Verification: if verifying work history is part of your credentialing/compliance, use this section to verify the work history entry.
  • Date Called: enter the date the facility was called.
  • Note: enter a note if applicable.
  • Verified: check this box if the information was verified.
  • Click OK.
  • If needed, you can upload documents about this work history entry.
